Job Description

  • Open your home to individuals with intellectual disabilities or brain injuries, ensuring it meets safety and accessibility requirements.
  • Create a welcoming and inclusive environment that promotes the individual’s comfort and sense of belonging.
  • Assist individuals with activities of daily living, such as grooming, dressing, bathing, and meal preparation.
  • Administer medications and provide any necessary medical support as outlined in the individual’s care plan.
  • Foster a caring and nurturing relationship with the individual, promoting their emotional well-being and self-esteem.
  • Encourage social engagement and community integration, helping the individual build relationships and participate in community activities.
  • Collaborate with the individual’s support coordinator and healthcare professionals to implement their individualized service plan (ISP).
  • Document progress, observations, and any incidents, and communicate regularly with the support coordinator and program administrator.
  • Maintain a safe and clean living environment that complies with health and safety standards.
  • Be prepared to respond to emergencies and ensure individuals’ safety.
  • Support the individual in developing and enhancing life skills, including communication, self-care, and independent living skills.
  • Provide opportunities for personal growth and self-advocacy.
  • Maintain accurate records of daily activities, medical administration, and progress reports as required by the HCBS waiver program.
  • Work collaboratively with the individual’s case manager, healthcare providers, and other professionals involved in the individual’s care.
  • Must have a safe and accessible home environment that meets Iowa HCBS waiver program standards and settings.
  • Strong commitment to providing compassionate care and support to individuals with intellectual disabilities or brain injuries.
  • Ability to follow care plans and medication administration procedures.
  • Excellent communication and interpersonal skills.
  • Willingness to complete required training and ongoing professional development.
  • Must be able to pass criminal history, motor vehicle, child abuse, dependent adult abuse, and sex offender registry background checks.
